Please click on the link below to access the Early Alert Form through PowerCAMPUS Self-Service.  Please be as detailed as you can, taking advantage of the fields available on the form.

Generally, use this form when a student has missed a full week’s worth of classes and has not been responsive to your attempts to communicate with him or her.  If a student does poorly on an exam or a written assignment, please refer them to take advantage of the academic services in The Learning Commons.  If you have concerns regarding the student’s behavior, please submit an Alert.  If you are not sure whether to submit an Alert, it is better to submit than to not submit it.
