Required for Fall 2013! Insurance Information Form (photocopy of insurance card required) (PDF)

Letter from the Student Health Center

Dear Parent/Guardian:

The Thiel College Student Health Center provides treatment of minor illnesses and injuries; basic first aid supplies; blood pressure screening; non-prescription cold, sinus and flu medications; and is a source for referral to local health care providers and health agencies.

Thiel College carries a secondary insurance on all full-time students. The coverage is limited and the College does not make any payment decisions. Students must use their primary insurance to its maximum benefits before claims are submitted to the secondary insurance (HMO and PPO plans must be utilized and proper referrals obtained). It is the responsibility of the parent/guardian and the student to know how their primary insurance works and to obtain referrals, if this is a requirement. Failure to utilize primary insurances properly will result in decreased benefits.

Claim forms for Thiel’s insurance plan are available in the Student Health Center. If an injury and/or illness occurs, your son/daughter must notify the Student Health Center, complete a required claim form and provide any requested information to the insurance company and the Student Health Center. Insurance questions should be directed to Pam Despo, coordinator of Student Health Center, at 724-589-2195.

It is helpful to have the student’s insurance information on file in case the student needs off-campus medical or emergency care. The Student Health Center does not file the insurance claim form for your son/daughter, but will provide them with the information needed, if it is on file. Please include a photocopy of both sides of your insurance card. Primary insurance referrals are the student’s and/or parents responsibility to obtain. It is very important that students know how their insurance works in this area.

The medical information and physical exam form in your registration packet must be completed by you and your health care provider prior to arrival at Thiel in August. This is a requirement for all new students entering Thiel. This is a requirement for admission and without it you will not be permitted to participate in any sports programs. The state mandated meningitis vaccination form must also be completed and returned for all new students residing in college housing.

Returning students need to update their insurance information but do not need to get another physical exam done by their health care provider (providing they had one completed and on file at the Student Health Center when they came to Thiel). All athletes are required to have a yearly pre-athletic history and physical completed prior to all collegiate sports participation. The athletic trainer arranges these physicals and it is the athlete’s responsibility to make sure he/she is present at the pre-arranged time for completion of these examinations. The trainer’s phone is 724-589-2143.

Payment for the physical examination and immunizations is the responsibility of the parent/guardian and/or the student.

Incomplete forms will be returned to the student for completion. The Student Health Center services cannot be provided to students who fail to meet these requirements. It is also the student’s responsibility to update health records and insurance information as needed.

Please mail completed forms to: Mrs. Pamela Despo, RN, MSN; Coordinator, Student Health Center; Thiel College, 75 College Avenue, Greenville, PA 16125

Sincerely,

Pam Despo, RN, MSN Coordinator of Health Services
