These are the most commonly used forms to request the various services that we provide in the MIMC.

CAN-8 Assignments form

   Use this to make an assignment in CAN-8 for your students to complete in the MIMC

Media Digitization & Duplication Request form

   Use this form to submit a request for the MIMC staff to change the format of existing media. 

   Example: requesting that we convert a VHS tape that is not available in DVD format to DVD. . 

Media Reserves form

   Use this to put media on Reserve for your students to complete in the MIMC

Media Loan Request form 

   Use this to sign out a title in any format from the Media Library

MIMC Class Registration form

   Use this to register your classes for use of the MIMC      

Thiel College Loan Agreement

   Use this online form to submit and authorize an equipment loan request to the A-V Desk.
